Jacob’s family described him as someone who had a very kind heart. He loved to volunteer and help people whenever he could. Additionally, he was very close to his siblings and enjoyed playing with his nieces and nephew. He loved to joke around and debate issues that he was passionate about.
Jacob was preceded in death by his grandfather, Allen Hazelwood; grandmother, Linda Graves; great grandfather, James “Gabby” Brooks; great grandmother, Daisy Spurlock; and infant sister, June Elaine Hazen. He is survived by his father, Chris Hazelwood and his wife, Amanda; mother, Mary Hazen; brothers, Marcus Graves and Ayden Hazen; sisters: Paige Hazelwood, Zoehy Hazelwood, Meghan Graves and Morgan Krouse and her husband, Austin; paternal grandmother, Crystal Hazelwood; maternal grandparents, Randy and Mary Mayhugh; paternal great grandmother, Virginia Brooks; his very special best friend, Prince Williams; and many other loving family members and friends.
Jacob’s family will receive friends Saturday, March 20, 2021, from 11 am – 1 pm at Newcomer Cremations, Funerals and Receptions, 925 East Hanna Avenue, Indianapolis, IN 46227. A celebration of his life will follow at 1 pm at the funeral home. For the health and safety of all guests attending, face masks are required.
Jacob really enjoyed painting and art, so his family requests that donations of art supplies be made, in his memory, to the psychiatry unit at Sanford Health, 300 North Seventh Street, Bismarck, ND 58501.
